Please download the Vendor Requirements Packet and send the completed forms as well as a completed W9 and Certificate of Insurance (for vendors who provide services or perform work onsite) to the Finance Department email: Finance.Department@tulsalibrary.org.
All vendors must submit an IRS W9 form for your business.
